TiimiVerkko - osallisuutta verkkovalmennuksesta

Meneillään olevat kansalliset uudistukset sekä sosiaali- ja terveyspalveluiden että julkisten palveluiden digitalisaation osalta painottavat palveluiden asiakaslähtöisyyttä, oikea-aikaisuutta, saatavuutta ja vaikuttavuutta. Keskeisessä asemassa ovat myös eriarvoisuuden vähentäminen sekä resurssien tehokkaampi käyttö ja kustannustehokkuus.

TiimiVerkko hankkeen tarkoituksena on tukea työelämän ulkopuolelle jääneiden yksilöiden työllistymistä ja osallisuutta uudenlaisilla matalan kynnyksen digitaalisilla palveluilla. Hanke toteutetaan useamman tahon yhteistyönä, joka tukee moniammatillisuuden ja organisaatiorajojen ylittävän yhteistyön mahdollistumista. Hanke mukailee myös maakuntastrategian (Keski-Suomi 2040) tavoitteita asiakaslähtöisten ja ennaltaehkäisevien hyvinvointipalvelujen  kehittämiseksi.

TiimiVerkko hankkeen (01.09.2018 -31.12.2020) päätoteuttajana on Jyväskylän ammattikorkeakoulu ja osatoteuttajana Jyväskylän katulähetys ry. Hanketta rahoittaa Euroopan sosiaalirahasto (ESR) Keski-Suomen elinkeino-, liikenne- ja ympäristökeskuksen (ELY) kautta.

The idea to prepare the project has risen from the need discovered from the field of profession. The lack of remotecoaching and rehabilitation services (utilizing remote technology applications) for the unemployed has been noticed.These technologies could be utilized as additional support with face to face rehabilitation. There are challenges inreaching the customers due to long distances, poor access to destinations, social limitations and schedule problems.These challenges support the need for new forms of services. The ongoing national reforms, both in social and health services as well as in the digitalization of public services, emphasize the customer orientation, timeliness, availability and effectiveness of the services. Key elements include reducing inequality and more efficient use of resources.Remu-project develops new kind of remote rehabilitation services as part of the existing services that promote workand functional capacity. The target groups are the long-term unemployed, young people outside education andworking life, and those with a low labor market position. In addition, the target group of the project is the employees of
organizations involved in the development of remote rehabilitation services in project pilot units.

The main objective of the project is to develop low threshold remote rehabilitation services for people outside working life. The aim is to:
1. Take remote rehabilitation as part of the services of target customers, and to promote the participant´s ability to work and perform and to speed up their progress towards working life.
2. To develop the professional´s remote guidance and coaching skills so that customer resources are better identified and supported by more active communication, using digital tools.
3. Strengthen the digital skills of the target group and those who workwith them and encourage professionals to take digital solutions as part of their guidance and coaching work.
4. Support the introduction of digital solutions in the future social welfare and health care -model by developing newforms of cooperation and services multi-professionally.
5. Develop and model new rehabilitation services that combinepersonalized remote solutions into existing forms of guidance services.


The project involves the following set of operations:
1) Professional as client – self-experimenting remote rehabilitationapplications
2) Mastering digital skills and remote solutions - Strengthening of professionals´ and customers' expertise
3) Customer pilots for remote rehabilitation solutions
4) Development of remote rehabilitation services
5) Instilling the results
6) Assessment 
7) Communication.

The operations will result new low-threshold remote rehabilitation services, which may include multidisciplinary online coaching for individuals and groups, online rehabilitation directing the patient to self-care, a mobile resource supporting coaching method. The ability to work and perform improves for those participating remote rehabilitation, the rehabilitation process and progress towards working life speeds up. The digital skills of customers and employees improve and professionals' remote management and training skills are developed, so that the resources of the customers are better identified and supported by digital tools through multi-professional collaboration. In addition, an operating model for professionals is developed, for taking remote rehabilitation services fluently as part of their existing work.
